Deflated Breasts After Weight Loss Surgery- Corrected with Implants and Lift, NC

This patient had bariatric surgery and lost well over 100 pounds. As a result, her breasts deflated completely having at one time been a triple D bra and now just loose skin. She wished to have return to her larger breasts.. The patient underwent augmentation mastopexy with placement of 455 cc Sientra Gel implants in the submuscular position with a full anchor pattern mastopexy. These types of cases are quite challenging as the patients have an enormous amount of excess skin which has to be carefully tailored with proper implant positioning. Submuscular placement in patients with this degree of thin skin is necessary to provide proper coverage of the implant. In fact, many surgeons recommend soft tissue support as this loose skin is weak and the implants can easily stretch and fall creating implant deformities. Recently patients have been offered a variety of meshes including absorbable meshes like Galaflex or Durasorb or even biological meshes like Strattice. (which is actually pigskin without the cells). In this patient, Dr. Lyle performed de-epithelialization of the skin and used it to support the lower portion of the implant. In this way, no foreign material was necessary, and he used her own tissues to provide the strength to support this somewhat heavy implant. She was very pleased with the improvement.
