This 18-year-old man was embarrassed by his large breast appearance. He developed breast tissue or gynecomastia at age 13 but it never went away . He was teased in the locker room during gym class and sports practices. He had a combination of excess fat and breast tissue so a combination technique with liposuction and direct excision was used. He achieved a much smoother appearing chest and was no longer self-conscious. Gynecomastia is often treated with a hybrid approach where aggressive liposuction was done to the fatty component. The Vaser device is very helpful to reduce this tough fibrous tissue, but the glandular tissue does not come out very well by liposuction alone and often needs to be directly. This can usually be done with smaller Incisions around the nipple and sometimes with a technique known as the avulsion technique where the remaining breast tissue is manually removed through a very small incision basically tearing it away from the surrounding tissue after it is loosened up with liposuction. It generally takes several months for swelling to go down and during that timeframe restriction of chest exercises as necessary as well as compression garments to help reduce swelling. Patients are encouraged to increase development of the pectoral muscle to get the best outcome.
