This 46-year-old woman underwent prior submuscular saline breast augmentation. She developed ptosis and some asymmetry with her left breast being lower and somewhat fuller. She wanted to maintain full upper profile but somewhat smaller breasts with a lifted appearance. She underwent bilateral implant exchange to somewhat smaller implants, she went from a 400-cc saline prosthesis to a 375-cc gel prosthesis. She also had a vertical mastopexy performed with a little bit more removal on the left side than the right to correct asymmetry. She was very pleased with the improvements made with her somewhat smaller appearing and lifted breasts. Revisional augmentation mastopexy can be performed safely in most instances. Careful attention to blood supply of the nipples is important as previous surgery has disrupted normal anatomy to some degree. If the patient does not have too much sagging, a vertical type mastopexy can often be lesser scarring.
