This 38-year-old woman had saline implants placed above the muscle by her prior surgeon. She developed lateral displacement and sagging and could easily feel her implants. Dr. Lyle performed site change to a submuscular pocket and changed implants form 300 cc saline implants to 450 cc silicone gel implants. The result was a much more natural appearing and feeling breast. Her implants were much less palpable after the correction. Site change refers to surgically changing the location of the implant often from on top of the muscle also known as sub glandular to under the muscle. He can also refer to changes from under the muscle to on top of the muscle which can be done for overactivity of the pectoral muscle called animation deformity. When placing implants from under the muscle to on top of the muscle, there is often a need for additional surgical meshes to reinforce the soft tissues. Breast revision surgery can be quite challenging, and things are not so simple just to change the position of the implant. Drain tubes are frequently recommended for a brief period postoperatively as the additional dissection will cause more low-level bleeding which could increase risk of capsular contracture.
