This 55-year-old woman suffered from neck and back pain and shoulder grooving. She wore a 36H cup brassiere. She was referred for breast reduction by her primary care physician who had documented measures to alleviate neck and back pain such as nonsteroidal anti-inflammatories, muscle relaxants, support bras and physical therapy. None of these were effective in the long-term. She underwent bilateral breast reduction with removal of 680 g on the right breast and 750 g on the left breast. Breast asymmetry is very common in woman and also may contribute to worsening back pain as an asymmetric distribution of breast tissue can lead to more stress on one side of the trunk compared to the other. She had marked improvement in her neck and back pain after surgery with smaller, better shaped and lifted breasts. Breast reduction is one of the most satisfying procedures for patients as it has an almost immediate effect on improvement in neck and back pain. Many woman that have it performed in middle age wish they had done so earlier in life.
