This 45-year-old woman from Wake Forest , NC had large sagging breasts causing neck and back pain and made it difficult to get proper clothing fit. She had relief of symptoms with a vertical scar breast reduction removing over 350 grams of breast per side. What are the different incisional patterns of a breast reduction? The most common is the Wise pattern also called inverted T or anchor pattern. This is typically utilized for very large breasts with excess skin as it can be modified to account all different breast sizes. A vertical breast reduction or limited scar technique typically involves elevation of the nipple -areola with a scar around the new areola and a vertical scar going down to the breast fold. There is no long inframammary fold scar but there may be a short scar extending laterally to address skin excess. Incisional patterns restricted to the areola also called Peri areolar technique are less common as don't reposition the nipple very well and have limited ability to shape the breasts. This technique may be suitable for a small reduction in a small breasted woman.
