More and more women are requesting breast lifts without implants. Although breast implants are very helpful for improving upper pole fill, they can make the performance of mastopexy much more difficult. In fact, breast lift with implants has a fairly high revision rate -reported at up to 20% even by experienced plastic surgeons. When patients undergo a breast lift, the nipple is elevated to a more perky position, the breast is reshaped and breast tissue is repositioned in a higher location. With time, gravity takes over and the breast tissue can fall -resulting in loss of fullness in the upper chest area. Many plastic surgeons now utilize an absorbable mesh such as Durasorb® or Galaflex™ to support the breast tissue that has been repositioned during mastopexy. These are made of the same material that is used in absorbable suturesl but dissolve over time and allow for healing while resisting the forces of gravity during the early postoperative period. These measures are particularly helpful for patients with prior weight loss or very loose fatty breast tissue. This 56-year-old woman wanted somewhat smaller, perkier breasts and did not want breast implants. Dr. Lyle performed bilateral mastopexy with small reduction and areolar reduction. He utilized mesh support also known as an "internal bra". She was very happy with her new appearance, and it motivated her to lose weight as she felt much more comfortable with her new shape.
