This 40-year-old woman lost 90 pounds over 2 years by going to Weightwatchers and starting an exercise program. Her breasts essentially "deflated" and became quite droopy. During consultation, expressed an interest in getting "perkier". She wanted to keep a moderate cup size but no implant. A Wise pattern mastopexy was performed. One of the most common requests of patients with major weight loss is breast lifting. A mastopexy repositions breast tissue, elevates the nipple to the center of the breast and reduces the breast diameter resulting in better shape. The inverted T or anchor pattern is the most common technique utilized by plastic surgeons especially with patients with a major amount of excess skin. Reduced scar techniques such as lollipop lift or vertical lift are not as commonly utilized in this population because the excess loose skin cannot be removed efficiently.
