The majority of patients with breast augmentation ultimately will require some sort of reoperation . Breast implants are not lifetime devices and have predictable failure rates. Breast implant leakage, capsular contracture, and implant malposition all can lead to revisional surgery. This 53 -year-old woman presented to Dr. Lyle with concerns about her deflated left breast implant. She also had questions about her intact right implant. Over 22 years ago she had saline breast implants placed on top of the muscle by another physician. She recently developed a leak which was notable by an immediate deflation of her left breast. She was noted to have significant capsular contracture of her intact right breast implant which was quite firm had a globular appearance and was uncomfortable. She wished to maintain nice full volume so was very interested in implant exchange and in fact chose to have silicone gel breast implants. She was found to have bilateral capsular contracture with calcified capsules but no evidence of cancer or any other problems. Because of her thin tissue and because of her capsular contracture she had what is known as a site change from the sub glandular position to the sub muscular position. This is performed to reduce risk of future capsular contracture. She had 450 cc breast implants originally and they were replaced with 490 cc Mentor gel implants. She recovered very nicely and is very pleased with her soft natural appearing breasts.
