There are a variety of reasons that woman choose to have their breast implants removed. Capsular contracture, chronic breast pain and concerns about breast implant illness are among a few . Many women just wish to have the implants removed as they feel they no longer are attractive or are too big. This is common after women have gained some weight or after childbearing. Many also worry about having to have implants removed in later life. This 47-year-old woman had her implants in place for over 13 years and had developed some drooping of the tissue. She felt her breasts were too big and wished to have them removed with a breast lift. She was concerned that she would be too small so she did have some liposuction and fat grafting to the breasts to overcome some of the volume loss. She was found to have 330 cc saline submuscular implants. Fat transfer was conservative and taken from the abdomen and flanks. Breast lifting after explantation can be a challenging procedure as the blood supply to the nipple areolar complex has been compromised and the collapsed appearance of the breast can be difficult to shape into a smaller perkier breast. The patient desired a C cup having previously wearing a D cup and was quite happy with the improvements noted. Her implants did not have any problems nor did she have any capsular contracture but she just wished to have them removed and realized she would be even droopier without the implants.
