This 49-year-old woman had always wanted fuller breasts and finally decided to pursue augmentation. She chose smooth walled saline implants placed under the muscle. She went from a small B to small D cup and was pleased with the improved appearance. Choice between submuscular and sub glandular implants has to be discussed with your plastic surgeon. As the silicone gel implants have improved, more physicians are recommending sub glandular or "on top of the muscle" technique. However, there are some potential disadvantages of on top of the muscle technique including increased risk of capsular contracture, potential for interference with mammography and potentially more palpability. It is believed that as the silicone gel implants have improved in terms of their composition, there is less chance of encapsulation. Many surgeons believe placing it under the pectoral fascia also called subfascial technique may reduce encapsulation rates, but this has not really been borne out by studies
