Augmentation mastopexy or breast implant with lift is generally performed for patients who have sagging breast tissue and drooping nipples with inadequate breast volume. It is a more complex procedure than breast augmentation alone and involves incisions to elevate the position of the nipple and reshape the breast. The result is scars cannot be avoided. There are several options to consider aside from the implant of choice. Implant choice is usually a silicone gel implant because patients with breast ptosis usually do not have enough tissue to hide the unfavorable palpability of saline implants. One choice is implant location and most plastic surgeons place them in the subpectoral position although subglandular or subfascial can be considered. There are generally 3 types of incision pattern. Periareolar mastopexy is usually for patients with minor degrees of ptosis. The scar is restricted to the area around the areola but it does not shape the breast very much. Vertical lift or lollipop lift involves a scar limited to the incision down the middle and around the nipple perhaps with a short scar at the bottom and this is used very frequently in augmentation mastopexy. Finally, the Wise pattern or anchor pattern results in the inverted T scar but is most effective for very saggy breasts where a lot of excess skin is present. This 39-year-old women lost volume post-partum and wished to restore fullness. She had vertical lift with 330 cc gel implants.
