Biocell textured implants were recently recalled as they have had a rare association with ALCL-anaplastic large cell lymphoma, which is a neoplastic involvement of the breast implant capsule likely caused by the textured surface and its interaction with cells present in the capsule. This device was known to be a more aggressive texture which could become affixed to the surrounding capsule. Although cure rates with complete capsulectomy are very high it is a potentially fatal disease. This patient did not have signs or symptoms of a ALCL but did develop painful contracted breasts and had saline textured Biocell implants placed in the past. She desired breast lift and downsizing of her implants. She underwent a complete capsulectomy and during the surgery was found to have a double capsule which is a separate capsule that encases the implant but separate from the outer capsule which is affixed to the breast tissue and muscle. All of the capsule was removed. The capsules were submitted to pathology and found to be completely benign without any ALCL. The implants were in the sub muscular position ; she had an exchange with mastopexy and had silicone gel smooth walled implants placed. She was very pleased with the somewhat smaller and lifted appearance of her breasts. Patients with Biocell textured implants exhibiting potential symptoms of ALCL such as breast swelling/Fluid collection ,firm mass or new onset pain/contracture should consult a plastic surgeon.
