This 44-year-old woman has lost weight and noted significant droopiness of her breasts. She was tall- nearly 6 foot and found that her nipples were falling to her bellybutton. She desired a breast lift and some enhancement from her size 40C to a D cup. She underwent a full Wise pattern mastopexy with augmentation with submuscular 300 cc silicone gel breast implants. After surgery, she felt much more comfortable in her clothing and was able to fit her breasts into a D cup nicely. Augmentation mastopexy is a more challenging operation than mastopexy or breast implants alone. Patients with loose skin such as this one present more challengs as the skin laxity potentially will continue over time. Placement of too large of an implant should be avoided because the extra weight potentially could stretch the pocket and the breast skin over time.
