Many women desire improvement in the abdomen with simultaneous fat transfer to the buttock-also known as Brazilian Butt Lift (BBL). While this can be an effective procedure, the recovery from an abdominoplasty is not conducive for the optimal recovery from a fat transfer because of positioning in which there is increased pressure to the recently delivered fat. Patients desiring larger fat transfer probably should have this in a staged fashion. There are many factors in determining whether or not the patient is a good candidate for a combined procedure. Dr. Lyle does limit prone positioning to patients with BMI of 30 or less and recommends doing the transfer after proper weight loss. Each patient has to be individualized and certainly the expectations have to be tempered. If Liposuction is recommended during abdominoplasty, it seems logical to be able to use some fat to enhance the buttock. This decision is not always straightforward. This 41-year-old has 4 pregnancies and had gained some weight and wished to have abdominoplasty with liposuction. It was recommended that she wait to do fat transfer during a second procedure. She did have some shallow areas of the outer buttock and didn't want to be large- just a better shape. After performance of the tummy tuck- 8 months later she had Vaser assisted liposuction with fat transfer to improve her buttock shape. She was very pleased with the outcome and now has maintained a healthier weight .
