Many women desire improvement in buttock shape at the same time as they wish to undergo abdominoplasty. This is not always ideal as the recovery from a fat transfer to the buttocks is best done staying off of the buttocks- while recovery from abdominoplasty involves a great deal of time sitting on the buttocks which can reduce fat graft survival. This 42-year-old woman desired fat grafting to the buttock. She acknowledged that a tummy tuck would be in her future as well but wished to have the improvements in her buttock performed first. She underwent fat transfer to the buttock with liposuction of the flanks and some from the abdomen and inner thighs in her initial procedure. After a full recovery, she returned to have a full abdominoplasty with additional liposuction of the flanks. She was very happy with the outcome. Although it seems tempting to have everything done in one setting, there are limitations of performance of fat transfer to the buttock with simultaneous abdominoplasty. The staged approach allows for fine tuning of the initial liposuction with additional liposuction during the performance of the abdominoplasty. Often, patient desire more fat placed into the buttock and this can be done during the second surgery.
