This 30 year old woman had undergone 90 lb weight loss but still had a large amount of skin and excess fat in her abdomen and buttock. Although she wanted extensive liposuction, her problems of excess skin and fat would be better treated with excisional body contouring with some liposuction. A circumferential abdominoplasty or lower body lift was recommended with some liposuction of her thighs. Liposuction during these procedures can be done but for safely concerns, have to be limited given the extensive nature of the procedure. The patient was very happy with the improvement in her buttocks and abdomen. Further liposuction in the future can be performed to fine tune the result but without skin removal first- excess liposuction just leaves patients with more excess skin. The concept of staging a body contouring procedure- performing excisional surgery first followed by more liposuction is often recommended when one operation alone will not achieve the desired result.
