This 46-year-old woman had 2 previous breast augmentations with one performed with saline devices changed over to silicone gel. Her last surgery was about 10 years ago she was never happy with her breasts and had developed some breast ptosis and had some pain on her right breast. She decided upon removal of her implants without replacement with mastopexy. Because she had relatively large implants, she was worried that she would be too small, so she agreed upon some conservative fat transfer at the time of implant removal. Explantation with mastopexy can be a complicated procedure where special care and consideration of blood supply to the nipple must be taken. Much of the breast tissue is preserved in order to achieve an adequate volume and if there is not enough, fat transfer can be useful but never makes up the volume of the original implant. Dr. Lyle performed liposuction of the abdomen with a C-section scar revision at the time of implant removal of her 350 cc breast implants. Techniques such as auto augmentation flaps which utilized the existing breast tissue to augment the shape of the now deflated breast are often necessary. Dr. Lyle utilizes the Viality ™ system which is a sterile lipoaspirate harvesting system which results in higher rates of fat retention. The fat must be placed primarily in the subcutaneous position which is often limited in these procedures. This patient had about 100 cc added per breast. She was very happy with the outcome.
