This 43-year-old mother of 2 had prior Roux-en-Y procedure for weight loss done over a year ago with resulting 105 pound weight loss She had pendulous large breasts and desired smaller lifted breasts and had significant amount of loose skin of her abdomen and some in the flank areas. She wished to fill out her buttock as well. She underwent near circumferential abdominoplasty, actually 270° Tuck which helps to elevate her buttocks somewhat and flanks. She had some fat transfer performed to the buttock and then the completion of the circumferential abdominoplasty with breast lifting. Performance of fat transfer at the time of the abdominoplasty is somewhat controversial. Many surgeons refused to perform this procedure. There are certain disadvantages of performing fat transfer at the time of abdominoplasty. The primary one is the inability to offload pressure on the newly transferred fat during the recovery of abdominoplasty as it involves sitting or laying on the buttock which is not ideal. Dr. Lyle will consider some degree of fat transfer during abdominoplasty but counsels his patients that the results are not going to be nearly as effective as when BBL is done alone. Although liposuction is not as commonly done during post-bariatric procedures, there is usually always some degree of fat removal that will improve the results. Utilizing the fat to improve buttock projection does make some sense. Additional future fat transfer can be considered.
