This 42-year-old woman had lost 190 pounds after bariatric surgery. She noticed a deflated flattened shapeless breast that occurred after surgery with loss of volume and flaccid loose skin extending into the bra roll area. She desired fuller volume and reshaping of her sagging breasts. She underwent bilateral augmentation mastopexy with placement of 325 cc silicone gel breast implants in the submuscular position with a full Wise pattern( anchor scar) mastopexy. The result was restoration of nice breast volume and removal of the loose skin and better shape of her breast Augmentation mastopexy after bariatric surgery can be a challenging procedure. Many patients have enough breast volume to have a nice shape and breast size with mastopexy alone. However, if volume loss was significant and implant is beneficial to improve upper pole volume. Because the loose skin of the breast never fully gets back to normal. A full mastopexy is generally necessary. This involves a higher degree of skin removal and the skin reshaping.
