Understanding the Risks and Potential Complications of Plastic Surgery and How to Reduce Them
From Dr. Glenn Lyle, North Carolina
As a board-certified plastic surgeon in Raleigh, North Carolina, my top priority is your safety and well-being. I want you to feel confident and fully informed as you consider plastic surgery. While the results of plastic surgery can be life-changing, it’s essential to understand that no surgery is without risks.
I’ve written this article to help you understand the potential complications, the measures I take to minimize these risks, and the steps you can take to ensure a safer and smoother recovery.
General Risks of Surgery
All surgeries, including plastic surgery, come with some level of risk. However, I take every precaution to ensure these risks are minimized and that your procedure is as safe as possible.
- Infection: Infections after surgery are rare, but I take this risk very seriously. I perform all procedures in accredited facilities that follow strict sterilization and infection control protocols. After surgery, I provide detailed instructions on how to care for your incisions and sometimes prescribe antibiotics when necessary to prevent infection. Interestingly, antibiotics are rarely recommended after surgery. Usually they are just administered in the IV prior to surgery as a one time dose.
- Bleeding and Hematomas: While minor bleeding is normal, excessive bleeding can lead to complications like hematomas. To minimize this risk, I use meticulous surgical techniques and may place drains when necessary to prevent fluid buildup. It’s important for you to avoid certain medications, like aspirin or blood thinners, before surgery, as they can increase the risk of bleeding.
- Anesthesia Complications: Serious reactions to anesthesia are very rare. I work with highly trained certified anesthesia providers who monitor you closely throughout the procedure. Your medical history will be carefully reviewed before surgery to identify and address any potential risks. Higher risk patients have their procedure performed in the hospital setting.
- Blood Clots (DVT and PE): Prolonged immobility during recovery can lead to blood clots forming in the legs, which can sometimes travel to the lungs. To reduce this risk, I encourage light movement soon after surgery, recommend compression garments, and even compression devices at home and, in some cases, prescribe medication to prevent blood clots. Despite these precautions, clots and pulmonary emboli can occur and are emergency situations that need immediate treatment in a hospital setting.
- Scarring: Scarring is a natural part of the healing process, but I take great care to place incisions in discreet locations and use precise closure techniques to minimize their appearance. I also provide guidance on scar care, such as using silicone gels and silicone tapes and avoiding sun exposure, to help scars fade over time. Laser treatments are sometimes recommended to treat scars that take longer to fade.
Procedure-Specific Risks
Every type of plastic surgery has its own unique risks. Here’s what you need to know about some of the most common procedures I perform:
Breast Surgery (Augmentation, Reduction, Lift)
- Implant Complications: For breast augmentation, complications like capsular contracture (hardening around the implant) or implant rupture are possible. I use high-quality implants and precise placement techniques to reduce these risks. Insertion funnels, antiseptic and antibiotic irrigation are used intraoperatively to reduce the potential for infection which can come from the bacteria in your skin and breast tissue itself- usually not outside contaminants. Capsular contracture is your own body's response to the implant but could be caused by excess blood around the implant after placement, or even minor degrees of bacteria - present in your skin or breast tissue that can attach to the implant and create a biofilm or bioslime that might not cause infection but induces you own healing mechanisms to produce excessive scar.
- Breast Implant Leakage/ Rupture: Leakage of either saline or silicone gel from implants is rare in the first 5 or so years after implantation but the rate slowly increase the longer that the implant is in place. At 10 years, less than 10% of women experience a leak. The devices are getting better and the implant manufacturers offer a warranty that covers a new implant and some financial help if the implant is found to be leaking.
- I also educate my patients on signs to watch for and schedule regular follow-ups to monitor implant health.
- Changes in Nipple Sensation: Temporary sensitivity changes are common, and permanent loss of sensation is rare. My surgical techniques prioritize preserving the nerves when at all possible to minimize this risk.
- Breastfeeding Challenges: If you plan to have children in the future, I discuss how surgery might affect your ability to breastfeed. In most cases, I use techniques that preserve the milk ducts and nipple function but there is often a potential for the inability to breast feed after more invasive procedures.
Body Contouring (Liposuction, Tummy Tuck, Body Lift)
- Contour Irregularities: Achieving smooth, even contours require precision and experience. I use advanced liposuction techniques and take great care to create balanced results. Even in the best of hands, irregularities are possible and may show up years later after surgery.
- Seromas (Fluid Build-Up): To prevent fluid buildup, I often place drains and recommend wearing compression garments after surgery. If a seroma does develop, I can quickly and easily manage it in the office with painless aspiration.
- Delayed Healing: Proper nutrition, hydration, and following my post-operative instructions can help your body heal more efficiently. If you have underlying health conditions like diabetes, I’ll work with you to manage these factors before surgery. Smoking cessation is must and even past smoking history can affect wound healing.
- Numbness/ Sensation Changes: All body contouring procedures involve separation of the skin from underlying tissue or at least trauma to the tissues in cases such as liposuction. Nerve endings can be cut or bruised leading to numb areas or uncomfortable sensations in the skin .In some cases, like the lower abdomen in a tummy tuck, these changes may be permanent but most of these sensory changes improve over time.
Facial Surgery (Facelifts, Eyelid Surgery, Rhinoplasty)
- Nerve Damage: Temporary numbness or tingling is common after facial surgery. I use delicate surgical techniques to protect your facial nerves and minimize the risk of permanent damage. Permanent nerve damage is rare but can occur .
- Asymmetry: While I strive for balance and symmetry, achieving perfect symmetry is not always possible. During surgery, I carefully evaluate and adjust to create the most harmonious and natural-looking results.
- Skin Healing Issues: Rarely, the skin may not heal properly or lose circulation. To prevent this, I plan incisions carefully and ensure blood flow to the area remains intact. Smoking significantly increases the risk of poor healing, so I strongly advise patients to quit smoking well before surgery.
Healing Complications to Watch For
Even after a successful surgery, certain issues can arise during the healing process. My team and I are here to support you every step of the way:
- Infection: Keep an eye out for redness, warmth, or unusual drainage around your incision sites. If you suspect an infection, contact me immediately so we can address it early.
- Excessive Swelling or Bruising: Swelling and bruising are normal after surgery but should improve gradually. I provide guidance on elevating the surgical area and using cold compresses to reduce swelling. I also recommend oral homeopathic agents like Arnica montana or Bromalain ( pineapple derived) to help reduce bruising. I also use tranexamic acid to tumescent fluid and local anesthetics which helps localized clotting.
- Seromas and Hematomas: If you notice unusual fluid buildup or swelling, let me know. These can often be managed with simple drainage techniques.
Long-Term Considerations
Plastic surgery results are not static, and it’s important to understand how they may evolve over time:
- Aging and Weight Changes: Your body will continue to age, and weight fluctuations can impact the results of your procedure. Maintaining a stable weight and healthy lifestyle will help preserve your results.
- Implant Longevity: If you have breast implants, remember that they may need to be replaced over time. I’ll discuss implant maintenance with you and help you plan for future care.
- Scarring: While scars typically fade over time, some patients may develop raised or thick scars. I offer treatments like steroid injections or laser therapy to improve their appearance if needed. Usually silicone based scar tapes and creams are recommended in the early post op period
Your Role in Reducing Risks
As a patient, you play an essential role in ensuring a safe and successful outcome. Here are some steps you can take:
- Follow Instructions: Carefully follow all pre- and post-operative instructions I provide. This includes avoiding certain medications, refraining from smoking, and attending follow-up appointments.
- Communicate Openly: Be honest about your medical history, medications, and lifestyle habits during our consultation. This helps me plan your surgery safely.
- Prepare for Recovery: Arrange for help at home, stock up on supplies, and give yourself plenty of time to rest and heal.
- No Smoking!

Your safety is my highest priority. I take the following steps to minimize risks and ensure the best possible outcomes:
- Board Certification: As a board-certified plastic surgeon, I stay current with the latest safety protocols and surgical advancements. The American Board of Plastic Surgeons requires ongoing safety education to maintain certification. I comply with all these requirements.
