Concern
Undesirable excess fat deposits can occur despite weight management, regular exercise and healthy lifestyle. This is often referred to as lipodystrophy and refers to unwanted fat deposits typically located in the abdomen and flanks thighs and back. While some modalities like cryolipolysis ( Coolsculpting) can be effective, results are usually not dramatic. Patients with good skin tone are the ideal candidates for liposuction and there are several types of liposuction such as power assisted liposuction and vibrational-assisted liposuction which helped the surgeon extracted that more efficiently. Vaser liposuction uses ultrasonic energy to shatter fat cells preserving the underlying collagen structure and may have some ability to cause some skin tightening. Other techniques such as the SAFE technique uses multiple steps to achieve similar results but in some patients loose skin off and after aging and pregnancies is hard to overcome.
This 47-year-old woman is quite fit but had difficulty with fatty deposits on her abdomen and was hopeful to avoid abdominoplasty. She was worried about skin laxity with too much liposuction which can be a problem
Surgery
The patient had Vaser -assisted liposuction with removal of over 1 L of fat from her abdomen and flanks as well as Renuvion; which is a specialized radiofrequency device used during liposuction to achieve more skin retraction. The radiofrequency energy targets helium gas creating plasma energy which helps to shrink collagen fibers exposed by liposuction with resulting tightening of the skin. It is not necessary for all cases but for patients with looser skin or areas susceptible to laxity he can be a very effective modality to improve the results of liposuction. The device is inserted into the same small incisions used to suction the fat. Dr. Glenn Lyle is one of the first doctors to use this in the Raleigh-Durham-Chapel Hill area.
Result
At 3 months she is showing nice improvement in contour and fat reduction. Most skin tightening devices used during liposuction take up to 6 months to see maximum improvement so she can expect even more toning during that time. Other noninvasive devices like Sofwave and RF microneedling can be effective for further skin tightening.
