This 69-year-old woman had initial treatment of a right capsular contracture by her original plastic surgeon. Her right side improved but he did not treat the left side which resulted in grossly asymmetric breasts with obvious capsular contracture on the left side.
Solution
This patient underwent a left capsulectomy with mesh internal bra. This is a procedure to reduce the chance of recurrent encapsulation. The results show nice improvement and better symmetry.
When a patient develops a unilateral capsular contracture, this could be a sign of a ruptured implant or it may have been because of a seroma or hematoma or other inciting inflammatory issue. The problem -when treating one side- is that it is very difficult to get completely symmetric breasts as the opposite side often has a milder degree of contracture and the breasts are almost never the same. This case demonstrates an exaggerated case of this where the left breast, despite being the exact same size implant as the right breast looks much smaller because of the encapsulation. In these cases, the implant is usually exchanged and patients will almost always benefit from a temporary drainage tube to reduce the chance of fluid collections which can cause a recurrent problem.
