This 31-year-old mother of 2 children had prior subglandular saline implants which were overinflated by her previous surgeon. She had severe rippling and easily palpable "fake" appearing breasts. She sought help form Dr Lyle who corrected things by changing to a subpectoral pocket and using a silicone gel implant. He also used allograft to thicken and reinforce the tissue. She had major improvement in her appearance. Her tissue was so thin that additional procedures may be necessary to decrease rippling and palpability further. She was very happy with the improvement achieved. Implant rippling occurs when folds in the breast implant are visible through the skin. This is more common in saline breast augmentation but can occur with silicone gel breast implants. Submuscular positioning reduces rippling, and it is also more common in very thin patients with very little breast tissue. Newer generation breast implants have a more cohesive or jellylike material resisting folds in the implant. Each breast implant manufacturer offers different versions of implants with different fill volumes or actually different silicone material. It is important to discuss these options with your plastic surgeon
