This 30 year old woman noted deflation of her breast size after giving birth to two children. She wore an A cup and wanted to be a full C cup. She chose silicone gel breast implants. She underwent bilateral sub muscular placement of Mentor smooth walled moderate plus 375 cc gel prostheses. She was very happy with her breast appearance after surgery and wore a full C cup bordering on a D cup. Dr. Lyle prefers sub muscular placement for the majority of his patients although some cases benefit more from sub glandular. Benefits of sub muscular positioning include less capsular contracture, less palpability of the implant, less interference with mammography and a less globular appearing breast. Potential downsides include- animation deformity ( movement of the implant with flexion of the pectoralis muscle), and possible drooping of the nipple over the implant( ptosis) if you have preexisting loose skin or ptosis.
