Breast augmentation performed simultaneously with breast lifting is called augmentation mastopexy. It is a more challenging procedure than breast augmentation alone. Many women come in for consultation requesting breast augmentation to improve their sagging or ptotic breast tissues. In some situations, they are disappointed to find out that their plastic surgeon recommends a mastopexy with the augmentation. This recommendation is based on the degree of ptosis of the breast tissue which is determined by the position of the nipple in relation to the inframammary fold as well as the degree generalized laxity of the breast tissue. Breast augmentation alone can lift the breast profile and nipple position in some circumstances as long as the degree of breast ptosis is minimal. However attempts at using an implant alone to lift the ptotic breast will often end in disappointment with low positioned nipples. Subglandular or subfascial positioning may help the situation but not in all cases. Placement of silicone breast implants either above or below the muscle at the time of mastopexy is a popular procedure especially for those with weight loss or significant ptosis particularly after pregnancy. This 45-year-old woman had lost weight and developed significant breast drooping and underwent augmentation mastopexy with placement of 565 cc breast implants. Submuscular positioning was utilized. She was very pleased with the improved upper pole and nice breast shape.
