This 37-year-old woman wanted a tummy tuck and improvement in her buttock appearance. Combining abdominoplasty with fat grafting to the buttock can be problematic since the recovery of a BBL requires avoiding pressure on the buttock while abdominoplasty recovery requires laying on your buttock. Because many abdominoplasties incorporate liposuction as part of the contouring, it makes some sense to do some fat grafting with the understanding that there may be a lesser success rate of the transferred fat. This that would be discarded otherwise. Special care during the transfer was necessary to avoid intramuscular injections. In the past intramuscular injections were recommended but are felt to be a source of potential lethal comp case fat embolism. Using ultrasound to guide placement of the fat is now recommended. The overall result was a much better abdominal appearance and better buttock contour. Aggressive liposuction the flank areas off and improve the shape of the buttock without any fat transfer.
