Gynecomastia or male breast enlargement comes in all forms. It can be as little as a small bud of sub-areolar breast tissue which causes nipples to protrude up to a fully formed female appearing breast. Surgical strategies must take this into account. Patients may need a formal excisional breast reduction with resulting scarring. Sometimes liposuction alone can treat the deformity. When patients have had significant weight loss, skin laxity prevents adequate retraction and shrinking of the skin and usually requires some degree of skin excision and resultant scarring. This patient was a 27-year-olld gentleman who had lost 100 pounds through dietary changes and lifestyle changes to include more exercise. He noted significant drooping of his breasts with enlargement with his breasts hanging flaccidly off of his now more developed pectoralis muscle. He was noted to have a symmetry with the left breast side droopier ,looser and larger. He was hopeful to have surgery to improve this without incisions however removal of the excess breast tissue without skin excision would result in even worsening skin laxity. He had peri -areolar Incisions with small extensions to remove skin redundancy. All attempts were made to reduce scar length and visibility but some had to be accepted. He had removal of excess breast tissue with preservation of his nipple in a higher more athletic position. Small scars were still present but major improvement in contour was achieved.
