Many factors have to be considered when determining the breast implant choice. Choosing an appropriate size has to be done in consultation with the plastic surgeon. Soft tissue characteristics such as how thick the brea tissue is, pliability of tissues, breast dimensions such as breast diameter, nipple to fold distance and sternal notch to nipple distance are all taken into account and ultimately matched to the proper implant based on the implant volume and profile. Even with the exact planning, intraoperative decisions have to be made as accurate preoperative planning although absolutely necessary, is not always absolute. Dr. Lyle uses 3D imaging provided by the Crisilix App as well as 3-dimensional planning to arrive at appropriate range of implant choices. This 22-year-old woman desired moderate increase in breast size from her small A cup to a larger B or small C cup. Because of her thin tissue, submuscular position was desired and she chose silicone gel implants. 3D imaging suggested implants between 250 and 275 cc were most appropriate. She had a fairly high nipple position and often if implant is too large or wide it can cause a bottoming out type deformity or high riding nipple. This was avoided by placement of a high-profile Allergan SSF 265 cc breast implant. An inframammary fold approach was used. Dr. Lyle uses the Keller funnel to insert the implant. The patient was very pleased with breast size and shape- now wearing a small C cup bra.
