This is a 47-year-old woman who underwent a gastric bypass 8 years prior to surgery. She had 100 pound weight loss and now has a body mass index of 25. She had some loose skin of her abdomen that primarily bothered her with some extension laterally and desired improvements not only in the abdomen but in the flank areas. She underwent circumferential abdominoplasty which is a preferred technique when skin laxity extends circumferentially. It does involve a scar entirely around the torso and is reserved typically for weight loss patients, post-bariatric surgery patients or patients with significant skin laxity. It does have the advantage of elevating the buttock and addressing the lateral thighs more effectively than standard abdominoplasty. This patient has maintained a healthy weight since surgery. The postoperative pictures are almost 5 years later. She has gained a small amount of weight but generally has remained very pleased with her contour.
